Coaches MUST submit this form IF your relay members differ from the regional meet to state meet. PLEASE NOTE: 4 athletes are required. The additional 4 are for alternates. The athletes do not have to be listed in running order. Please be prepared to fill in ALL requested information. You must complete and submit all information at the same time in order for your form to be processed properly. Be sure to review your information carefully before clicking Submit. For any changes after April 29th at 12:00pm, changes will be made on-site at packet pick-up.
